2008 LGBTQ Community Health Survey

The LGBTQ Community Health Assessment Survey will allow us to determine future health and wellness programs such as free screenings, tobacco quit groups, weight and nutrition classes, etc. 

Check back after May 10!  The survey will be accessible online after that date.  In the meantime, be sure to fill it out in person when you have the opportunity, and enter the drawing to win one of five donated $50 gift cards to Circuit City or Macy's.  Drawings for the gift cards will occur on June 30, 2008.
